Payment is due in full on the day of service.
This savings plan cannot be used in conjuction with another dental plan.
This savings plan cannot be used in conjuction with any office promotions.

Doctor exams (1-2/year)

20% off all other treatments

Adult Membership Plan (13 & older)

For patients with regular cleaning and the absence of periodontal disease.

Perio Membership (Members with periodontal disease)

20% off all other treatments

For patients enrolled in active periodontal maintenance due to prior treatment for periodontal (gum) disease.
